I, ____________________________ parent/legal guardian of ____________________________ , who is below 18 years of age, willingly provide my consent on behalf of my child to Synergie , the implementation partner of the Saksham Scholarship Program on behalf of Mahindra and Mahindra Financial Services Limited , to collect, process, use, and retain my child’s personal information, including name, mobile number, address, Aadhaar number, age, gender, and other relevant details , for the purpose of administering, verifying, and facilitating participation in the Saksham Scholarship Program .
I understand that the information provided may be used for scholarship eligibility verification, communication, documentation, program administration, and other activities directly related to the Saksham Scholarship Program. By providing my digital consent, I confirm that I am the parent/legal guardian of the child named above and that I voluntarily provide consent for the collection and use of my child’s personal information for the purposes stated above. I further confirm that the information provided by me is accurate to the best of my knowledge and that I have the authority to provide this consent on behalf of the child.
I understand that:
My personal information will be used to understand my family annual income as per eligibility criteria for the program.
Synergie will use this data only for program-related communication, research, and impact assessment, and only for such time as specific to the program.
My personal information will be kept secured and will not be shared with any third party without my permission, except to the extent strictly required for the purpose of the scheme/program and as required under applicable laws or any regulatory or legal obligation.
My personal information will be shared with Mahindra and Mahindra Financial Services Limited, under whom this CSR (Corporate Social Responsibility) initiative has been undertaken. I understand that Mahindra Finance will be a separate Data Fiduciary and not a Data Processor.
Synergie will implement appropriate technical and organizational measures to protect my personal information from unauthorized access, disclosure, alteration, or destruction.
Synergie will retain my personal information only as long as necessary to fulfil the purposes outlined in this consent form, as required by applicable laws and/or legal and regulatory obligations, or in accordance with my specific consent.
I understand that Synergie is an independent implementation partner and is seeking this consent from me in Synergie’s own capacity as a Data Fiduciary (“Data Fiduciary” as defined under applicable data protection laws).
I am sharing my information, including personal information, voluntarily, and I understand that I can withdraw my consent at any time by informing a representative of Synergie.
Further, I understand that if I refuse to give my consent or withdraw my consent anytime later, such refusal or withdrawal of my consent may affect the ability of Synergie to provide me with services and/or benefits under any scheme(s).
If the beneficiary filling this consent form is a minor (below 18 years of age), this consent may be provided by the beneficiary's parent or legal guardian on the beneficiary's behalf. By signing this form, the parent or legal guardian confirms that they are authorized to provide such consent and that the information provided is true and correct to the best of their knowledge.
If I have any questions or concerns about this consent form or the processing of my personal information, or if I wish to exercise any rights which I have under the applicable data protection laws, I understand that I can contact Synergie as per the contact information provided on the application portal under the ‘Need Help’ section.
